Free USMLE Step 1 Practice Questions

360 board-style questions with full explanations, written in the same clinical vignette format you’ll see on exam day. Pick your difficulty and answer at real exam pace — 90 seconds per question.

How this quiz works

The 360 questions are split into three sets of 120 — easy, medium, and hard. If you’re early in your prep, start with easy to build a foundation and some confidence. The hard set is closer to what you’ll face in the toughest stretches of the real exam: longer vignettes, subtler distractors, and connections that cross organ systems.

After you submit each answer, you’ll see why the correct answer is right and — just as importantly — why each wrong answer is wrong. Most students learn more from understanding the distractors than from getting questions right, so don’t rush past the explanations. Each one ends with a short teaching point worth remembering.

Your score, misses, and average time per question are summarized at the end. If you’re consistently running out the timer, that’s a signal to work on question pacing — one of the most common and fixable weaknesses in Step 1 prep.

Every question includes an explanation of the correct answer, a breakdown of why each other option is wrong, and a key teaching point to help the concept stick.

On the real USMLE Step 1, you get roughly 90 seconds per question. Practicing at that pace from the start builds the timing instincts you need on exam day. If time runs out, you still see the answer and full explanation.

The questions span the major Step 1 disciplines — pathology, pharmacology, physiology, biochemistry, microbiology, anatomy, and behavioral science — all written as clinical vignettes.

As many times as you like. Questions are shuffled on every attempt, so the order is always different.
